Perspectivas da aplicação das conotoxinas bloqueadoras de canais para cálcio dependentes de voltagem no trauma medular

Resumo:
O bloqueio dos canais para cálcio dependentes de voltagem é uma
estratégia importante no tratamento do trauma medular, pois previne o influxo
exacerbado do cálcio que participa ativamente em processos neurodegenerativos
agudos, resultando em neuroproteção com melhora das funções neurológicas.
Dentre esses bloqueadores, as toxinas de caramujos marinhos são peptídeos com
adequada estabilidade estrutural, estudadas pelas ações específicas em canais
iônicos e receptores que interferem diretamente na liberação de neurotransmissores e na neuromodulação dos
neurônios motores e sensitivos da medula espinal. Elas já são utilizadas no
tratamento de desordens neurológicas e mostram-se promissoras no
desenvolvimento de novas terapias para o trauma medular. Portanto, objetivou-se
discorrer sobre a fisiopatologia do trauma medular e a possível utilização
terapêutica das toxinas de caramujo marinho, atuantes nos principais canais
para cálcio dependentes de voltagem.

treatment, because it prevents the
exacerbated calcium infl ux which participates actively in acute neurodegenerative processes,
resulting in neuroprotection with
improvement of neurological and electrophysiological functions. The cone snail toxins are peptides
with adequate structural stability,
which have been studied by specifi c actions on ion channels and receptors that directly
interfering in the release of neurotransmitters and neuromodulation of sensory
and motor neurons of the spinal cord. They are already used in the treatment of neurological disorders and appear to be
promising in the development of new
therapies for spinal trauma. Therefore, it was aimed to discuss the pathophysiology of spinal
cord trauma, and possible
therapeutic use of marine snail toxins that acts in voltage-dependent calcium
channels.

Resumo: Relata-se o primeiro caso no Brasil de tumor venéreo transmissível (TVT) vaginal com metástase para o sistema nervoso central (SNC), além do envolvimento do tecido cutâneo, ocular e pulmonar e descreve a resistência do TVT ao tratamento com vincristina. O TVT é uma neoplasia contagiosa ocorrendo, principalmente, em locais em que não ocorre um controle populacional de cães, e atualmente tem sido demonstrada resistência destas células tumorais ao tratamento convencional com vincristina. Foi atendida uma fêmea canina, sem raça definida, com cerca de cinco anos de idade, pesando 9,2kg, com histórico de sangramento vaginal há mais de seis meses. Após diagnóstico citológico de TVT foi recomendado tratamento com quimioterápico. Houve resistência a quimioterapia com sulfato de vincristina, preconizando a utilização de quimioterápico doxorrubicina. Suspeita-se que o desenvolvimento do tumor no sistema nervoso central possivelmente tenha ocorrido devido à doxorrubicina não ter capacidade de atravessar a barreira hematoencefálica, aliado à resistência à vincristina.
Abstract: The first case in Brazil of vaginal TVT with metastasis for the central nervous system is reported, as well as the presence of cutaneous, eye and lung metastases and describes the resistance of the TVT to treatment with vincristine. The TVT is a contagious neoplasm that occurs mainly in places where there is no dog population control, and currently, resistance of these tumor cells to conventional treatment with vincristine has been demonstrated. A mongrel female dog of about five years old, weighing 9.2 kg, with a history of vaginal bleeding for more than six months was attended. After cytological diagnosis, treatment with chemotherapy was recommended. There was resistance to chemotherapy treatment with vincristine sulfate, advocating the use of doxorubicin. It is suspected that tumor development in the central nervous system possibly occurred because the doxorubicin was not capable of crossing the hematoencephalic barrier, along with resistance to vincristine.

Background: Cervical fractures in dogs occur most commonly in the cranial region, mostly requiring surgery. Various types of implants are being used while fixation using plate is poorly described in the literature. The plate and screw types are a limiting factor since they can lead to loss of stability due to loosening of the screws. The use of locking plates has been advocated, which does not allow movement between the screw-plate-bone, providing extreme stability and rigidity to the system. This study describes the use of locking bone plate to stabilize axis fracture in a dog and the results obtained with this technique.
Case: A 9-month-old male Poodle presented due to a history of trauma to the cervical spine caused by a fall of an object. Upon physical examination, the patient was alert and physiological parameters within the normal reference limits. However, neurological examination showed tetraplegia, hyperreflexia, preserved nociception and much cervical pain. There were no changes in the cranial nerves test. Cranial cervical lesion was initially suspected and the patient was rigidly fixed on a flat surface. The radiographic examination showed a fracture of the second cervical vertebra (axis) with anatomical axis deviation, and the patient was referred for surgery. After anesthesia, the dog was positioned dorsal decubitus and rigidly fixed on the operating table with the thoracic limbs pulled caudally. The surgical approach of the cranial cervical spine started through the ventral access until complete visualization of the fracture line and the caudal portion of the axis body. After perfect apposition and alignment of the bone fragments, rigid stabilization was performed using locking plate and screws. The patient had a favorable neurological recovery, and five days after the surgery, no change was observed in locomotion and postural reactions, besides the absence of neck pain.
Discussion: The implant used in this study was small and displayed a good fi t along the body axis. The locked system allowed the bolt head to lock in the hole of the plate, forming a bone-screw-plate unit that prevents its failure. In our case, the tip of the caudal screws protruded approximately two millimeters within the spinal canal, without any apparent effect on the outcome. In a previous study, in which plates were used for ventral fixation and stabilization of the atlantoaxial joint, the screws also protruded into the vertebral canal without causing any problems, presumably because the cervical vertebral canal is wider than the diameter of the spinal cord at this location. Failure rate of up to 44% has been reported for all the processes of atlantoaxial ventral fixation if the surgery is deemed successful when resolution of neurological signs occurs, and there is no need for further surgery. According to this, the present case can be considered successful
taking into account the clinical outcome after surgery, the rapid reduction of pain, return to ambulation and the absence of neurological deficits. We conclude that the locking plate was a viable alternative to other fixation techniques for fractures involving the second cervical vertebra in small animals since it allowed relative stability of the fracture and an excellent neurological recovery of the patient.

AVALIAÇÕES PRÉNATAL ULTRASSONOGRÁFICA E RADIOGRÁFICA NO DIAGNÓSTICO DE ANENCEFALIA EM CÃO - RELATO DE CASO

A anencefalia é uma malformação da cabeça e está entre as malformações que envolvem defeitos no tubo neural do embrião. Em humanos os defeitos do tubo neural são malformações comuns, entretanto em medicina veterinária estas malformações são consideradas raras. O exame ultrassonográfico é muito utilizado na rotina clínica veterinária no que diz respeito à gestação. Anomalias do concepto detectadas pelo exame ultrassonográfico pré-natal tem sido descritas em cães. O objetivo deste trabalho é descrever o resultado ultrassonográfico realizado na avaliação pré-natal e os achados radiográficos de um feto, onde foi possível diagnosticar anencefalia. Conclui-se com o presente estudo que a ultrassonografia torna-se útil na avaliação gestacional e quando associada com a radiográfica permitiram identificar anencefalia. Neste caso, demonstrou-se a enfermidade debilitante em um cão e assinala a importância de estudos com malformações embrionárias nesta espécie para que se possa determinar a real importância destas malformações e se há fatores desencadeantes com o intuito de minimizar problemas financeiros e emocionais com a perda de conceptos.

Resumo
As neoplasias com origem no canal medular são infrequentes na clínica veterinária de pequenos animais, porém quando presentes podem causar sinais neurológicos importantes. Anatomicamente estas neoplasias podem ser classificadas em extradural, intradural-extramedular e medular. As neoplasias extradurais estão localizadas fora da dura-máter, porém podem causar compressão da medula. Os condromas são neoplasias benignas, caracterizadas pela formação de tecido cartilaginoso sendo rara a sua localização no canal medular tanto em medicina humana como na medicina veterinária. O objetivo deste trabalho é descrever um caso de condroma extradural lombar em cão em que a descompressão cirúrgica e retirada da massa permitiu a resolução dos sinais clínicos de paralisia com retorno da função dos membros pélvicos do paciente.
Abstract
Neoplasms originating in the spinal canal are uncommon in small animal veterinary clinic, but when present can cause significant neurological signs. Anatomically, these neoplasms can be classified as extradural, intradural-extramedullary and medullary. Extradural neoplasms are located outside the dura mater, but they can cause compression of the spinal cord. The chondromas are benign neoplasms characterized by the formation of cartilage and is rarely located in the spinal canal in both human and veterinary medicine. We describe a case of lumbar extradural chondroma in a dog that surgical decompression and removal of the mass allowed the resolution of clinical signs of paralysis with return of function of the hind limbs of the patient.

O traumatismo cranioencefálico (TCE) em cães é causa comum de disfunção neurológica na rotina clínica veterinária. O objetivo deste trabalho foi avaliar os casos de traumatismo cranioencefélico em cães atendidos entre 2008 a 2010, com foco nos sinais clínicos e neurológicos, dosagem da glicemia imediatamente após o trauma e nos dias subsequentes, as lesões concomitantes, o tratamento instituído, a evolução do caso e avaliação da utilidade da Escala de Coma de Glasgow Modificada (ECGM) no prognóstico destes pacientes. Para isso foram acompanhados 32 cães com trauma cranioencefálico em diferentes graus de severidade. A causa mais comum de TCE foi o atropelamento e a maioria dos animais acometidos pesava menos de 5 kg. Os fatores peso, tempo entre ocorrência do trauma e atendimento, presença de lesões associadas em outros sistemas, medicação administrada previamente ao atendimento, local do sistema nervoso central comprometido, presença de hiperglicemia no atendimento inicial e uso de manitol não interferiram na sobrevivência dos pacientes. A pontuação ECGM no atendimento inicial interferiu significativamente na evolução dos pacientes, pois entre os animais sobreviventes, a maioria chegou ao hospital com pontuação boa a moderada e no grupo dos não sobreviventes metade apresentou pontuação grave no atendimento inicial. Entretanto, sobrevivência e boa pontuação final na ECGM não significaram capacidade funcional como animais de estimação, pois vários pacientes apresentaram sequelas neurológicas importantes.
Prospective study of 32 dogs with traumatic brain injury
Traumatic brain injury (TBI) in dogs and cats is a common cause of neurologic dysfunction in the veterinary clinic’s routine. The aim of this study was to access the head trauma cases in dogs treated from 2008 to 2010, focusing on the clinical and neurologic signs. We also evaluated the glucose blood levels immediately after the trauma and in the following days, the presence of concomitant injuries, the efficacy of treatment instituted, the evolution of the case and the utility of the Glasgow Coma Scale Modified (ECGM) on the prognosis of these patients. To accomplish that, 32 dogs with head trauma in several degrees were evaluated. The most common cause of brain injury was being hit by a car and most of affected animals weighted less than 5 kg. The factors as weight, time between the occurrence of trauma and treatment, presence of associated injuries in other systems, medication administered prior to treatment, location of the lesion in the central nervous system, the presence of hyperglycemia in the initial care and use of mannitol did not predict survival of patients. The Modified Glasgow Coma Scale score (MGCS) in the initial care significantly interfered with the patient outcome, because among the group of survivors most come with a good score in the initial care, and in the group of no survivors, half presented serious score in the initial care. However, survival and good final score in ECGM did not meant functional capacity as pets, since many patients had significant neurological sequelae

Choroid plexus papilloma in a Rottweiler: computed tomographic, gross morfological and histological features

ABSTRACT
Among the tumors affecting the choroid plexus in dogs, the papilloma ranks second place in incidence after cell carcinoma tumors. Presumptive diagnosis can be made through imaging methods, such as computed tomography and magnetic resonance images. Definitive diagnosis of plexus choroid tumor is based on histopathological findings. This report presented the tomographic features of the brain in a 6-year-old intact female Rottweiler with choroid plexus papilloma. The computed tomography showed right lateral ventricle enlargement, midline deviation and an enhanced mass into the postcontrast phase. At necropsy, a mass on the floor of the right lateral ventricle was observed, associated with important ventricle dilatation. The histopathological analysis demonstrated the presence of neoplastic cell forms with papillary projections. The computed tomography proved to be an effective tool in the presumptive diagnosis of this kind of cerebral disorder.
Papiloma do plexo coroide em um cão da raça Rottweiler: aspectos tomográficos, macroscópicos e histológicos
RESUMO
Dentre os tumores que afetam o plexo coroide em cães, o papiloma figura como o segundo tipo de maior incidência, antecedido apenas pelo carcinoma. O diagnóstico presuntivo pode ser elaborado por meio de métodos de imagem, como a tomografia computadorizada e a ressonância magnética. O diagnóstico definitivo de tumor do plexo coroide é estabelecido com base nos achados histopatológicos. Relatamos os aspectos tomográficos do crânio em uma fêmea de 6 anos, inteira, da raça Rottweiler, com papiloma do plexo coroide. A tomografia computadorizada revelou dilatação do ventrículo lateral direito, desvio da linha média e a presença de uma massa, que sofreu realce na fase pós-contraste. À necropsia, foi observada uma massa sobre o assoalho do ventrículo lateral direito, associada à importante dilatação ventricular. A análise histopatológica demonstrou a presença de células poligonais neoplásicas, arranjadas em papilas longas. A tomografia computadorizada apresentou-se como uma ferramenta eficaz no diagnóstico presuntivo desse tipo de alteração cerebral.

RESUMO
Obtiveram-se imagens tomográficas sem alterações da cabeça de cães, visando à familiarização com os aspectos normais do encéfalo e correlacionaram-se os achados com a respectiva anatomia da região estudada. Várias estruturas anatômicas foram identificadas, tais como o parênquima dos lobos frontal, parietal, temporal e occipital, a fissura longitudinal, o sistema ventricular, o cerebelo, o bulbo olfatório, o corpo caloso, o diencéfalo, a ponte, a medula oblonga e o sulco quiasmático, de forma direta ou relacionando-as com estruturas vizinhas que auxiliaram na sua identificação.
Computed tomography of the dog's brain: normal aspects and anatomical correlation
ABSTRACT
Normal tomographic images of dog's heads were obtained, aimed to familiarize them with the normal aspects of the brain and correlate these findings with the relevant anatomy of the region studied. Several anatomical structures, such as the parenchyma of the frontal, parietal, temporal and occipital lobes, the longitudinal fissure, the ventricular system, the cerebellum, the olfactory bulb, the corpus callosum, diencephalon, the pons, the medulla oblongata and the chiasmatic sulcus were directly identified or were related to neighboring structures which helped in their identification.

RESUMO
Objetivou-se descrever a ocorrência de lesão iatrogênica meningomedular em um cão da raça Dachshund, macho, oito anos de idade, apresentando tetraparesia flácida há dois meses, que foi submetido à espondilectomia ventral para tratamento de extrusão crônica de disco intervertebral cervical. Durante remoção do material de disco herniado com removedores de tártaro e pinça hemostática, observou-se severa hemorragia dos seios venosos, descolamento meníngeo com laceração medular e imediata queda dos parâmetros cardiovasculorrespiratório, com evolução para a morte ante a ausência de respostas às medidas de ressuscitação cardiopulmonar.
ABSTRACT
Our objective was to describe the occurrence of an iatrogenic meningo-medullary lesion and the trans-surgical complications in an eight-year-old male Daschund, which presented flacid tetraparesis for two months, and underwent ventral spondylectomy to treat a chronic cervical intervertebral disc extrusion. During the removal of the herniated disc with dental picks and hemostatic forceps, we observed meningeal dislocation with medullary laceration and an immediate reduction in cardiovasculatory and respiratory parameters, which progressed to a cardiorespiratory arrest that was non-responsive to medication and finally led to the death of the patient.

RESUMO
Relata-se um caso de lipoma de células fusiformes infiltrativo diagnosticado em cadela com sinais neurológicos de ataxia proprioceptiva, reação postural ausente no membro pélvico direito, paraparesia fracamente ambulatória e dor à palpação sobre as vértebras torácicas craniais. A mielografia demonstrou compressão extradural do lado direito sobre a quinta vértebra torácica. À necropsia foi observado neoplasma que invadia o canal vertebral na quarta e quinta vértebras torácicas com compressão acentuada da medula espinhal. Microscopicamente, foram observados adipócitos neoplásicos bem diferenciados,com áreas de células fusiformes, diagnosticado como lipoma de células fusiformes infiltrativo. A avaliação por imuno-histoquímica, com anticorpo anti-CD34, revelou positividade principalmente nas áreas fusiformes do lipoma.
ABSTRACT
A female dog was referred presenting neurological signs of proprioceptive ataxy, proprioceptive deficit in the right pelvic limb, mild ambulatory paresis and spinal pain during the palpation in thoracic vertebrae. It was observed a right extradural compression of spinal cord over fourth and fifth thoracic vertebrae by spinal myelography. During necropsy evaluation, it was observed a neoplasm which infi ltrated in the spinal canal in the fourth and fi fth thoracic vertebrae with marked compression of the spinal cord. Microscopically there were well differentiated neoplasic adipocytes, with areas of spindle cells, diagnosed as infiltrative spindle cell lipoma. Resumo: Em animais é baixa a incidência de arterosclerose e hipertensão primária. Devido a tal característica, infarto cerebral é incomum nos mesmos. Entretanto, com o avanço das modalidades de imagem, doença vascular está sendo reconhecida com maior frequência na medicina veterinária. Doença cerebrovascular pode ser subdividida em infarto e hemorragia, embora as duas categorias se interponham no caso de infartos hemorrágicos. Assim sendo, o objetivo deste artigo é descrever as manifestações neurológicas associadas a acidente vascular (infartos) em uma gata de dois meses de idade, sem raça definida e domiciliada. Na avaliação neurológica observou-se inclinação de cabeça, tetraparesia, déficits proprioceptivos nos quatro membros e diminuição do reflexo pupilar a luz. Além disso, os problemas neurológicos foram agudos e progressivos. Na necropsia macroscopicamente detectou-se hemorragia e necrose no mesencéfalo e cerebelo. No exame histopatológico confirmou-se a presença de necrose liquefativa no mesencéfalo e cerebelo. Os sinais neurológicos associados com os achados patológicos são sugestivos de infarto levando a anóxia provavelmente devido à oclusão vascular.
Caudal brain infarctions in a kitten – case report
Abstract:: Stroke is uncommon in animals compared with humans because of the lower incidence of atherosclerosis and primary hypertension. However with advanced imaging, vascular disease is being recognized with increasing frequency in veterinary medicine. Cerebrovascular disease can be subdivided into infarction and hemorrhage, although the two categories overlap in the case of hemorrhagic infarcts. The aim of this article is to report the neurological manifestations associated with stroke (infarctions) in at two-month old, domestic shorthair cat. Neurological evaluation revealed head tilt, tetraparesis, proprioceptive deficits in all four limbs, and decreased pupillary light reflex. Further, manifestations of neurological dysfunctions were acute and progressive. At the necropsy, grossly there were hemorrhage and necrosis at mid-brain and cerebellum. Histopathology confirmed liquefactive necrosis at the mid-brain and cerebellum. The neurological manifestations associated with the pathological findings are suggestive of an anoxic infarction probably due to vascular occlusion.

Resumo: O tratamento médico para a hidrocefalia inclui a administração de medicamentos para limitar a produção do fluido cerebroespinhal (FCE), resultando em redução da pressão intracraniana (PIC). Este trabalho descreve os achados clínicos em um cão com hidrocefalia congênita não responsiva ao tratamento médico convencional com esteróides, mas que apresentou boa resposta à associação omeprazolesteróides. O omeprazol pode diminuir a produção de FCE em cerca de 26% de acordo com estudos experimentais realizados com cães saudáveis. Porém, o uso do omeprazol em ensaios clínicos com cães enfermos, como os animais hidrocefálicos, não é descrito. Os resultados deste trabalho sugerem que o omeprazol pode ser empregado em associação ao corticóide para melhorar o estado neurológico em cães com aumento da PIC devido à hidrocefalia.
Abstract: Medical therapy for hydrocephalus includes the administration of medications to limit the production of the cerebrospinal fluid (CSF) resulting in reduced intracranial pressure (ICP). This report describes the clinical findings in one dog with congenital hydrocephalus that was unresponsive to conventional medical treatment with steroids, but demonstrated good response to omeprazole when this drug was added to the steroid. Omeprazole might decrease the CSF production by about 26% according to experimental studies with healthy dogs, but the usage of the omeprazole in clinical trials with affected dogs such as hydrocephalic animals is lacking. The results of this report might suggest that omeprazole can be used added to steroids to ameliorate the neurological status in dogs with increased ICP by hydrocephalus.

RESUMO: Devido à importância do trauma cranioencefálico (TCE) em pequenos animais, o presente
trabalho teve por objetivos discorrer sobre a fisiopatologia da afecção, os procedimentos terapêuticos pré-hospitalares e hospitalares, além de considerações relacionadas aos cuidados no transporte e manejo inicial do paciente imediatamente após o trauma. O profissional deve identificar o TCE pré-hospitalar e tratar o paciente como um indivíduo politraumatizado, incluindo os cuidados com a imobilização. Na terapia hospitalar os procedimentos de craniotomia são importantes, sobretudo para retirada de coágulos. Além disso, o uso de glicocorticóides precisa ser evitado devido aos efeitos secundários. Por outro lado,
associações terapêuticas como a do manitol com a furosemida aumentam as perspectivas de sucesso.
TRAUMATIC BRAIN INJURY IN SMALL ANIMALS
ABSTRACT Due to the importance of traumatic brain injury (TBI) in small animals, the aim of this review
was to discuss the pathophysiology of the disease, the pre-hospital and hospital therapeutic procedures, as well as considerations related to transport and initial care of the patient immediately after trauma. The professional must identify the pre-hospital TBI and treat the victim as a polytraumatized patient, including immobilization. In hospital therapy the procedures of craniotomy are important, especially to remove blood clots. In addition, the use of glucocorticoids must be avoided because of side effects, but combination therapies such as mannitol with furosemide increase the probability of success.

Clinical and Pathological Findings of Necrotizing Meningoencephalitis in a Maltese Dog

Abstract: A 2-year-old, intact female Maltese dog was presented to the veterinarian with a history of acute neurological signs. On neurological examination the dog showed deficit of mental status (apathy and depression), seizures, constant howling, head turn and compulsive circling to the right side and falls to the left side. The treatment protocol using prednisolone (for seizures remission) and cyclosporine (initiated in the chronic stage) did not stop the progression of the disease and euthanasia was elected 65 days later. Necropsy revealed mild cerebral asymmetry, and in the frontal (more affected) parietal and occipital lobes of the right hemisphere there were friable, depressed and yellowish areas characterizing malacia. The left contralateral frontal lobe was edematous and slightly yellowish. At histopathology, the lesions were characterized by marked, multifocal to coalescing necrotizing meningoencephalitis, characterized by focally extensive areas of malacia, especially in the cortex of the frontal and right parietal lobes. Extension of lesions to white matter was observed only in the caudal region of the right frontal lobe. Plasma cells and lymphocytes infiltration was observed around vessels, leptomeninges and in the neuroparenchyma. In addition, the non-cavitation areas were also characterized by neuropil vacuolization, neuronal necrosis, neuronophagia, astroglyosis with various gemistocytes, endothelial hyperplasia and hypertrophy. The immunohistochemical analysis showed predominance of CD3 positive T lymphocytes in proportion to CD79 positive cells. Clinical signs, character and distribution of neurological lesions were compatible with necrotizing meningoencephalitis (NME). This condition, initially reported only in Pugs, currently affects other breeds and attention should be given to the differential diagnosis with other neuropathies in dogs.

Abstract: In the last 20 years, an unknown neurological disease affected cat populations in the Western Border Region of the State of Rio Grande do Sul, Brazil. The onset of the disease was characterized by tail paralysis, followed by progressive paraparesis in the pelvic limbs, difficulty in ambulation and proprioceptive ataxia. After long prolonged clinical courses (12-24 months), when then affected cats became severely paraparetic and start to develop pressure sores do to decubitus, they were destroyed by the owners. At necropsy there were variable degrees of skeletal muscle atrophy of the pelvic muscles and some degree of reddening of the meninges at the level of T10-L7, due to the presence of a myriad of small blood vessels, as a typical varicose lesion that resembled a vascular hamartoma. Histologically, such lesions consisted of distension of the subarachnoid space due to a collection of dilated, blood-filled, tortuous blood vessels the lumina of which were occasionally partially or completely occluded by thrombi. Those varices were randomly surrounded by a lymphocytic or granulomatous inflammatory infiltrate with small foci of mature eosinophils.
In the lumina of these varicose venules cross and longitudinal sections of nematode parasites could be observed. Based on the morphology of these parasites, in their anatomical localization (meningeal blood vessels) and in the species (cat) affected the nematode was identified as Gurltia paralysans. This paper describes detailed aspects of the epidemiology, clinical disease and pathology of this intriguing feline myelopathy and the definitive diagnosis of the condition: Feline crural parasitic paraplegia, a disease first described in Chile in the 1930’s and now, for the first time, in Brazil.
Resumo: Nos últimos 20 anos, uma doença neurológica desconhecida acometeu populações de gatos da Região da Campanha do RS e tornou-se um desafio diagnóstico para os veterinários locais. Os gatos afetados desenvolviam inicialmente paralisia da cauda, seguida de paraparesia progressiva nos membros pélvicos, alteração da marcha, posição plantígrada e ataxia proprioceptiva. Após longos períodos de evolução clínica (12-24 meses), quando se tornavam marcadamente paraparéticos e começavam a apresentar escaras de decúbito, eram sacrificados pelos proprietários. Na necropsia, demonstravam graus variados de atrofia dos músculos pélvicos e algum grau de avermelhamento das meninges entre T10 e L7, devido à presença de miríades de pequenos vasos sanguíneos, uma típica lesão varicosa, semelhante a um hamartoma vascular. Histologicamente, tais lesões consistiam de distensão do espaço subaracnóideo por vasos sanguíneos dilatados e tortuosos, repletos de sangue e, ocasionalmente, de trombos, que ocluíam parcial ou totalmente seus lúmens. Essas varizes venulares eram aleatoriamente circundadas por infiltrado inflamatório granulomatoso ou linfocítico com pequenos focos de eosinófilos maduros. No lúmen das vênulas varicosas havia secções transversais e longitudinais de parasitos. Com base na morfologia desses parasitos, em sua localização anatômica (vasos sanguíneos meníngeos) e na espécie afetada (gato), o nematódeo foi identificado como Gurltia paralysans. O objetivo desse trabalho é apresentar em detalhes os aspectos epidemiológicos, clínicos e anatomopatológicos dessa intrigante mielopatia de gatos e estabelecer definitivamente seu diagnóstico definitivo: paraplegia crural parasitária felina, uma doença descoberta no Chile, na década de 1930, e agora, pela primeira vez, descrita no Brasil.

Mielomalácia hemorrágica progressiva em 14 cães

RESUMO: A mielomalácia hemorrágica progressiva (MHP) é uma afecção rara e fatal, em que ocorre necrose aguda isquêmica e progressiva do parênquima da medula espinhal levando à liquefação da mesma. Pode ocorrer após extrusão do disco intervertebral, trauma medular ou embolismo fibrocartilaginoso. Este estudo teve como objetivo avaliar casos de mielomalácia hemorrágica progressiva em cães atendidos no Hospital Veterinário da Universidade Estadual de Londrina entre os anos 2000 e 2011, realizando-se a análise dos prontuários de atendimento e acompanhamento dos casos. Os animais do presente estudo atendiam a alguns critérios de inclusão, como histórico de paraplegia com sinais de neurônio motor superior, piora dos sinais progredindo para tetraplegia flácida, alterações clínicas progressivas e/ou alterações nos exames complementares. Foram analisados os aspectos epidemiológicos (raça, idade e sexo), clínicos (evolução dos sinais clínicos e neurológicos), laboratoriais (análise do líquido cefalorraquidiano), Radiográficos (radiografias simples e contrastadas) e o tempo decorrido desde o início dos sinais clínicos até óbito ou eutanásia. A raça Teckel foi a mais acometida (43%), a média de idade foi de 5,04 anos e no atendimento inicial a síndrome toracolombar grau V foi a alteração mais encontrada, além de hiperpatia e progressão cranial da diminuição do reflexo cutâneo do tronco. Em sete cães a causa da MHP foi a doença do disco intervertebral toracolombar, em um cão a causa foi o trauma medular, em dois cães a MHP foi decorrente de linfoma e em quatro cães a causa provável foi doença de disco intervertebral. Alterações na análise do líquido cerebroespinhal, na mielografia e na evolução dos sinais clínicos e neurológicos foram extremamente importantes para diagnosticar a MHP. Seis animais progrediram para tetraplegia e quatro cães já apresentavam tetraplegia flácida no atendimento inicial. Em outros quatro pacientes, a identificação de sinais sugestivos de MHP antes desta progressão levou à indicação de eutanásia. Como o prognóstico é ruim e ocasiona sofrimento ao animal, o clínico deve estar atento ao histórico de paraplegia com posterior mudança da síndrome de neurônio motor superior para neurônio motor inferior, diminuição do reflexo cutâneo do tronco cranialmente e presença de respiração abdominal, sendo que algumas alterações em exames complementares encontradas neste trabalho também podem auxiliar no diagnóstico precoce da MHP, como o líquido cerebroespinhal xantocrômico com aumento de proteínas, hemácias e pleocitose. Na mielografia o edema medular e a presença de contraste no interior do tecido nervoso, frente às alterações clínicas e liquóricas, são sugestivas de MHP.
Progressive hemorrhagic myelomalacia in 14 dogs
Progressive hemorrhagic myelomalacia (PHM) is a rare and fatal disorder which is characterized by acute and progressive ischemic necrosis of the parenchyma of the spinal cord, leading to its liquefaction. It may occur after intervertebral disc extrusion, spinal trauma or fibrocartilaginous embolism. The aim of this study was to evaluate cases of progressive hemorrhagic myelomalacia in dogs in the Veterinary Hospital of Universidade Estadual de Londrina between 2000 and 2011, through the analysis of medical records and following of cases. There were certain criteria to include a patient in this study, such a history of paraplegia with upper motor neuron signs, worse of signs progressing to flaccid tetraplegia, progressive clinical changes and/or changes in complementary exams. There were analyzed several aspects, such as epidemiological (breed, age and sex), clinical (progress of clinical and neurological signs), laboratory (cerebrospinal fluid analysis - CSF), radiographic (conventional radiography and contrasted) and elapsed time since the onset of clinical signs until death or euthanasia. The most affected breed was Teckel (43%), the average age was 5.04 years and the neurological syndrome observed initially was the thoracolumbar syndrome grade V. Another commons signs observed were hyperpathia and cranial progression of decreased cutaneous trunci reflex. In seven dogs the cause of the PHM was the thoracolumbar intervertebral disc disease, in a dog the cause was spinal cord trauma, in two dogs PHM was due to lymphoma and in four dogs the likely cause was intervertebral disc disease. CSF analysis, myelography changes and progress of clinical and neurological examinations were extremely important to diagnose PHM. Six animals progressed to tetraplegia and four dogs had already flaccid tetraplegia at the initial care. In four other patients, the identification of signs suggestive of PHM before this progression has led to indication for euthanasia. The prognosis is poor and causes animal suffering, so the clinician should be aware of the history of paraplegia with subsequent change of upper motor neuron syndrome to lower motor neuron, cranial decreased reflex panniculus and presence of abdominal breathing. Some alterations in complementary exams found in this study may also help in early diagnosis, as xanthochromic CSF with increased protein, erythrocytes and pleocytosis. Spinal cord edema and the presence of contrast within the nervous tissue together with clinical signs and CSF alterations are suggestive of PHM.

A luxação da articulação atlanto-occipital é considerada uma afecção incomum no homem e nos animais.
Radiografias laterais são recomendadas para o diagnóstico. No entanto, estão sujeitas a erros relacionados
ao ângulo de radiação, ao alvo da imagem, à distância e à sobreposição óssea. Objetivou-se neste relato
descrever os achados clínicos e radiográficos de dois cães com luxação traumática da articulação atlantooccipital que apresentavam tetraparesia, dor cervical cranial, incapacidade de elevar a cabeça e déficits de nervos cranianos. No primeiro animal, observaram-se deslocamento craniodorsal do processo articular do atlas em relação a um dos côndilos do occipital, ausência de sobreposição dos forames vertebrais laterais e sobreposição do côndilo do occipital ao processo articular do atlas, no lado direito, caracterizando uma luxação unilateral. No segundo animal, observou-se deslocamento craniodorsal dos processos articulares do atlas em relação aos côndilos do occipital, com sobreposição dos forames vertebrais laterais e ausência de visibilização dos côndilos do occipital em virtude da projeção cranial dos processos articulares do atlas em direção ao crânio, caracterizando luxação bilateral. Conclui-se que o exame radiográfico simples, nas projeções laterolateral e ventrodorsal, apesar da dificuldade de ser interpretado, é eficiente para confirmar o diagnóstico da luxação atlanto-occipital traumática, tanto a simétrica quanto a assimétrica.
Atlanto-occipital luxation is considered rare in both humans and animals. Lateral radiographs are recommended for diagnosis, however, errors may occur related to the angle of radiation, image target, distance and overlapping of bone. Our objective is to report the clinical and radiographic findings in two dogs with traumatic atlanto-occipital luxation, which had tetraparesis, cranial neck pain, and inability to raise the head and cranial nerve deficits. The first animal had a cranio-dorsal dislocation of the articular process of the atlas in relation to one of the occipital condoles, with no overlapping of the transverse foramens or overlapping of the occipital condile in relation to the articular process of the atlas, on the right side, which characterizes a unilateral luxation. The second animal presented with a cranio-caudal dislocation of the articular processes of the atlas regarding the occipital condoles, with overlapping of the transverse foramens and inability to visualize the occipital condoles due to the cranial advancement of the articular processes of the atlas towards the skull, which characterizes a bilateral luxation. We conclude that a simple radiographic exam, in lateral and ventrodorsal projections, though difficult to interpret, is efficient in confirming a diagnosis of traumatic atlanto-occipital luxation, both symmetric and asymmetric.

Porencephaly and cortical dysplasia as cause of seizures in a dog

Abstract
Background: Seizures are a common problem in small animal neurology and it may be related to underlying
diseases. Porencephaly is an extremely rare disorder, and in Veterinary Medicine it affects more often ruminants,with only few reports in dogs.
Case presentation: A one-year-old intact male Shih-Tzu dog was referred to Veterinary University Hospital with history of abnormal gait and generalized tonic-clonic seizures. Signs included hypermetria, abnormal nystagmus and increased myotatic reflexes. At necropsy, during the brain analysis, a cleft was observed in the left parietal and occipital lobes, creating a communication between the subarachnoid space and the left lateral ventricle, consistent with porencephaly; and also a focal atrophy of the caudal paravermal and vermal portions of the cerebellum. Furthermore, the histological examination showed cortical and cerebellar neuronal dysplasia.
Conclusions: Reports of seizures due to porencephaly are rare in dogs. In this case, the dog presented a group of brain abnormalities which per se or in assemblage could result in seizure manifestation.

Transcranial Doppler sonographic findings in granulomatous meningoencephalitis in small breed dogs

Abstract — Granulomatous meningoencephalitis (GME) is an acute, progressive, and often fatal inflammatory disease of the central nervous system, affecting mainly small and toy dog breeds. A definitive diagnosis of GME can only be achieved through histopathologic examination of samples collected after death. This retrospective study describes transcranial Doppler ultrasonography (TDS) findings in dogs with confirmed clinical histopathology of GME. Eleven dogs were selected for this study. Sonographic findings in B-mode demonstrated diffuse decreased brain parenchyma echogenicity in 9 dogs, ventriculomegaly in 8 dogs, brain atrophy in 4 dogs, and hyperechoic focal lesions in 6 dogs. Color Doppler imaging revealed more obvious vessels of the arterial circle in 10 dogs. Spectral Doppler examination was performed in 10 dogs to detect the 6 major cerebral arteries of interest. The examination showed normal and high resistive index (RI) values in the outlined arteries. The TDS findings were consistent with pathology found on postmortem examination

Abstract: Spinal cord injury is a common cause of neurological
dysfunction in dogs and cats. Lesions in these species are due to various types
of accident, which can cause sequelae that impair the patient for life as a
pet, or cause life-threatening injury. The main purpose of this work was the
accompaniment of animals with spinal cord trauma seen from August 2009 to November
2010 at the Veterinary Hospital of Universidade Estadual de Londrina, studying
the epidemiology and etiology, risk factors, spinal cord segments most
affected, outcome of conservative or surgical treatment, ratio of time of
patient care with recovery, efficacy and side effects of neuroprotective drugs,
complications, sequels and evolution of patients. During this period we
monitored 57 animals (48 dogs and nine cats). We observed a predominance of
males (68%) and indoors (79%). The main cause of injury was being hit by a car
(66%). The time between injury and first attendance was less than eight hours
in 42% of cases and more than a day in 51%. The spinal segment most affected
was the thoracolumbar (52%). Twenty animals were euthanized after the initial
attendance due to poor prognosis. The conservative treatment such as rest and/or external immobilization
was performed on 29 animals, and a good outcome was seen in 72.4% of this
patients: total functional recovery was observed in 17 (58.6%) animals and
partial functional recovery was seen in four (13.8%). Four animals did not
recover, and four animals died. Surgical treatment was performed in eight
patients, and three animals recovered, one patient did not recover and four
animals died or were subjected to euthanasia due to complications during
intraoperative or postoperative period. Conservative treatment was a viable
therapy, mainly in dogs with cervical
spinal cord trauma.
Resumo: O traumatismo da
medula espinhal é uma causa comum de disfunção neurológica em cães e gatos.
Lesões nestas espécies ocorrem devido a vários tipos de acidentes, podendo ocasionar
sequelas que prejudicam o paciente como animais de estimação ou lesões que
comprometam a vida. O objetivo principal deste estudo foi a monitoração
intensiva de animais com lesão medular atendidos entre 8/2009 e 11/2010 no
Hospital Veterinário da Universidade Estadual de Londrina, estudando os fatores
epidemiológicos e etiológicos, fatores de risco, segmentos medulares mais
afetados, resultados dos tratamentos conservativo ou cirúrgico, relação entre o
tempo de atendimento e a recuperação do animal, eficácia e efeitos colaterais do
succinato sódico de metilprednisolona, complicações, sequelas e evolução do
quadro. Durante este período foram acompanhados 57 animais (48 cães e nove
gatos). Observou-se predominância de animais machos (68%) e domiciliados (79%).
A principal causa de lesão foi o atropelamento (66%). O tempo entre o trauma e
o atendimento foi menos de oito horas em 42% dos casos e mais de um dia em 51%.
O segmento medular mais atingido foi o toracolombar (52%). Vinte animais foram
submetidos à eutanásia após o atendimento inicial devido ao prognóstico
reservado. O tratamento conservativo com
repouso e/ou imobilização externa foi realizado em 29 animais e um bom
resultado com esta modalidade foi obtida em 72,4% dos casos: houve recuperação
funcional total em 17 (58,6%) animais e parcial em quatro (13,8%). Quatro
animais não se recuperaram e quatro animais vieram a óbito. Oito pacientes
foram submetidos ao tratamento cirúrgico, sendo que três animais
recuperaram-se, um paciente não apresentou recuperação e quatro vieram a óbito
ou foram submetidos a eutanásia devido a complicações no trans ou
pós-operatório. O tratamento conservativo foi viável, principalmente em cães
com lesão medular cervical.

Clinical syndromes of nervous distemper in dogs initially presented without conventional evidences of CDV infection

Resumo: Apesar da prática de vacinação, o vírus da cinomose canina (CDV) ainda é um importante gente infeccioso que leva à doença neurológica na população canina em todo o mundo. Infelizmente, o diagnóstico clínico da encefalomielite pela cinomose é difícil nos casos apresentados sem evidências clínicas convencionais de infecção pelo CDV, tais como sinais sistêmicos e mioclonia. Portanto, o objetivo deste estudo foi avaliar os déficits neurológicos e as síndromes clínicas da apresentação neurológica da cinomose em cães portadores de doença neurológica apresentados na ausência de evidências convencionais de infecção pelo CDV. Foram prospectivamente acompanhados cães que apresentaram doença do sistema nervoso central, onde a abordagem diagnóstica ante mortem excluiu outras afecções que não cinomose (etiologia tóxica, traumática, degenerativa, neoplásica, etc), sendo incluídos neste estudo os animais que vieram a óbito (morte natural ou eutanásia, apesar do tratamento médico) e foram necropsiados. Dez de 35 cães avaliados foram diagnosticados no post-mortem com encefalomielite pelo CDV pela detecção de RNA viral pelo RT-PCR no tecido nervoso e observação de lesões neuroparenquimatosas compatíveis com cinomose. De acordo com os sinais neurológicos, a história clínica e a idade de apresentação, os cães foram agrupados em três síndromes clínicas da forma neurológica da cinomose: i) encefalite pela cinomose canina em cães imaturos (CDEID) (n = 3); ii) encefalomielite multifocal pela cinomose em cães adultos (MDEMD) (n = 6); e iii) síndrome similar a encefalite do cão velho (ODE-like) (n = 1). Os respectivos déficits neurológicos esperados para cada uma das síndromes aqui apresentadas foram discutidos.
Abstract: Despite large vaccination practice, canine distemper virus (CDV) is yet an important infectious agent that leads to nervous disease in canine populations worldwide. Unfortunately, the clinical diagnosis of distemper encephalomyelitis is often difficult in cases presented without conventional evidences of CDV infection such as systemic signs and myoclonus. Therefore, the aim of this study was to evaluate the neurological deficits and the clinical syndromes of nervous distemper in dogs suffering from neurological disease admitted in the absence of the conventional evidences of CDV infection. Dogs presented with central nervous disease in which toxic/traumatic or a CDV-free etiology could be excluded ante mortem were prospectively followed up and which that died (natural death or euthanasia, despite medical treatment) and necropsy was carried out were included in this study. Ten out of 35 evaluated dogs were post mortem diagnosed with CDV encephalomyelitis by both CDV RNA detection through RT-PCR assay in nervous tissue and observation of distemper-compatible neuroparenchymal lesions. According to the nervous signs, clinical history, and the age of presentation, the distemper dogs were grouped in three clinical syndromes of CDV encephalomyelitis: i) canine distemper encephalitis in immature dogs (CDEID) (n=3); ii) multifocal distemper encephalomyelitis in mature dogs (MDEMD) (n=6); and iii) old dog encephalitis (ODE)-like syndrome (n=1). The respective nervous deficits expected from each one of the syndromes herein presented were discussed.
Avaliação de um kit de imunoensaio cromatográfico para detecção do antígeno do vírus da cinomose em cães com sinais sistêmicos ou neurológicos da doença

A cinomose é uma doença multisistêmica altamente contagiosa, que pode levar a graves sequelas e até ao óbito. Diferentes tipos de exames podem ser usados para o diagnóstico ante mortem da cinomose, entretanto, devido ao curso imprevisível da doença, o diagnóstico final permanece incerto em alguns casos. O objetivo deste trabalho foi avaliar a eficácia do teste comercial de imunoensaio cromatográfico para detecção de antígeno (Ag) da cinomose em 33 cães com suspeita de cinomose canina, comparandose ainda a frequência dos sinais clínicos e neurológicos entre cães positivos e negativos. Em 27/33 o material foi coletado da mucosa ocular, em 5/33 foi coletado do LCR e em um animal o material foi coletado da mucosa ocular e do LCR. Em 14/33 cães foi realizada também a técnica da reação em cadeia da polimerase precedida de transcrição reversa (RT-PCR). O teste do Ag foi positivo em 7/13 casos confirmados, sendo que todos esses apresentavam sinais sistêmicos, porém em nenhum cão que apresentou apenas sinais neurológicos o teste do Ag foi positivo. Em seis cães negativos pelo teste do antígeno o resultado da RT-PCR foi positivo. Os resultados obtidos neste trabalho evidenciaram que o teste do Ag não auxiliou no diagnóstico da cinomose em muitos casos com sinais sistêmicos e neurológicos, permitindo ainda que vários casos apresentassem resultados falso-negativos.
